A Case of Malignant Lymphoma Converted to Acute Leukemia

Abstract

|
Here presented a case of malignant lymphoma who developed acute lymphoblastic
leukemia 1 month after the start of radiation therapy.
The patient was 16-year-old Chinese boy and was admitted to Severance Hospital
due to cough and joint pain. There were multiple cervical and inguinal lymphnode
enlargment, hepatosplenomegaly and mediastinal widening. Lymph node biopsy disclosed
malignant lymphoma, mixed cellular type.
Vincristine and cyclophosphamice were administered and mediastinal irradiation was
done for troublesome cough and dyspnea with some benefit. One month later, he
developed fever and frustration. Examination of bone marrow and peripheral blood
demonstrated a picture of acute lymphoblastic leukemia. He was treated with
6-mercaptopurine, cytosine arabinoside, methotrexate and prednisolon and has been in
hematologic remission state for 3 months.
Thereafter, CNS leukemia was complicated and it was treated in virtue of CNS
irradiation and intrathecal methortrexate. Two months later, there developed exacerbation
of leukemia and he died from upper gastrointestinal bleeding.
